CLIENT CONSENT FORM
Terapizy Teknoloji Anonim Şirketi (hereinafter referred to as the “Company” or “Terapizy”), acting as the data controller, requests your explicit consent for the processing and/or transfer of your personal data, except in cases where the conditions for processing personal data specified in Article 5(2) of the Personal Data Protection Law apply (e.g., explicit provision in laws, necessity for the establishment or performance of a contract, etc.), with respect to the following matters.

Within the scope of the Client Information Notice on Personal Data Protection shared with me by Terapizy,
Regarding the personal data provided by me during the conclusion of the contract and specified in the Client Information Notice,
I consent to:
- The collection and storage of data such as age, marital status, gender, sexual orientation, profession, the area in which therapy is sought (based on the provided response), sleep patterns, nutritional status, reasons for seeking therapy, the platform through which Terapizy was accessed, and my photograph data for the purpose of facilitating the selection of an appropriate therapist, and the transfer of such data to the therapist conducting the therapy session to provide them with preliminary information about me to enhance the efficiency of the therapy,
- The publication of my comments and ratings regarding therapies on Terapizy’s website and mobile applications,
- The transfer of my personal data to domestic and foreign companies providing information technology, cloud, and archiving services for the purposes of storage, archiving, and ensuring data security, as well as the transfer of such data abroad due to the storage of email contact addresses in data centers located overseas for email service infrastructure.
I provide my explicit consent by clicking the approval button.
I am aware that I may withdraw this explicit consent at any time, that such withdrawal will take effect prospectively, that all activities carried out based on my explicit consent will cease from the moment my withdrawal statement is received by the Company, and that my data will no longer be processed for the purposes stated above.
